Over the 20th century the trope migrated into cinema (examples include Dracula's Daughter, Hammer/Karnstein films and later exploitation cinema) and into contemporary media where it is sometimes subverted into sympathetic or queer‑affirming portrayals.

Descriptions vary by work, but literary prototypes like Carmilla emphasize feminine beauty and erotic allure — languid, burning eyes, intimate gestures, and kisses described in sensuous terms. Film adaptations range from suggestive comportment and invitation scenes to explicit eroticized violence (e.g., biting during sexualized encounters in mid‑20th century and exploitation films). Beyond attractiveness and predatory kisses/biting, sources do not ascribe a single definitive physical form to the trope.

Portrayals draw on general vampire conventions reframed through a sapphic erotic lens: feeding on human victims (often depicted as young women in narrative examples), seduction and intimate physical contact used to access victims, and the corruption or alteration of social roles (the vampire as corrupter of innocence). Some related feminine vampiric figures in other traditions (for example the pontianak) are noted as gendered avengers, but that is a separate folkloric motif rather than identical to the literary 'lesbian vampire'.
